Assessments & Reports

Assessments and reports at Reaura Therapy are approached with care, professionalism, and a strong focus on understanding the broader context of a person’s experiences, wellbeing, relationships, supports, and day-to-day functioning.

Reports are developed through a collaborative and systems-informed approach, recognising that emotional wellbeing, psychosocial circumstances, relationships, environments, and lived experiences can all impact a person’s functioning and support needs. The aim is to provide clear, thoughtful, and practical documentation that supports understanding, advocacy, service access, and meaningful support planning.

Assessments and reports may include:

  • psychosocial assessments

  • wellbeing and support needs assessments

  • documentation relating to wellbeing, functioning, and support needs

  • systems-informed support summaries

  • NDIS-related supporting documentation

  • advocacy and support letters

  • risk and protective factor considerations

  • collaborative support planning documentation

  • family and environmental context considerations

  • reports supporting referrals and service coordination

My Approach

My approach to assessments and reports is grounded in clarity, compassion, and understanding the experiences and circumstances that can impact wellbeing and daily functioning.

I value creating documentation that is thoughtful, person-centred, and reflective of the broader factors impacting a person’s life, while remaining practical, professional, and accessible for the supports and systems involved.
